Daily Nursing Services
Medical Case Management:
This service allows the Schooner Home Health RN to work directly with the tenant’s primary physician to address and manage all current health care needs, including evaluation of any significant changes in health status and coordination of any health services needed including in-home care, therapies and on-going medical appointments. Home Health RNs also to act as liaisons between tenant, family, physician and other outside agencies.
Schooner Home Health RNs can perform in-home nursing assessment visits and communicate results to the primary physician. They can also provide medication management including weekly medication set-ups and ordering of medication refills from pharmacy and lab management. This service includes arrangement of in-home lab services provided by Nordex laboratories as per MD orders as well as follow up on test results with the physician.
Medication Management:
We ensure accuracy and adherence by handling weekly medication set-ups, ordering refills, and assisting with the administration of all necessary prescriptions (including eye drops, inhalers, and insulin).
Lab Management:
We coordinate convenient in-home lab services based on physician orders, and promptly follow up to communicate the results to your doctor.
Assisted Living Services:
- Personal care services such as baths, showers, shampoos, dressing, grooming, toilet needs and changing bed linens
- Assist to apply support or compression stockings
- Nail care/manicures
- Assisting with exercises/ambulating
- Escorting to meals and/or activities
- Measuring blood pressure, pulse and temperature
- Making bed and tidying up apartment
- Extra loads of laundry
- Medication reminders
- Administration of eye drops, ointments, inhalers, nebulizer treatments, insulin and finger stick blood sugar testing
- Assisting with oxygen equipment
- Light meal preparation and social visits
- Emergency response
Wellness & Continuing Good Health:
- Twice weekly blood pressure clinics
- Podiatry clinics
